Palm Health SMS Consent — Public Opt-In Evidence
Public compliance evidence for the Palm Health SMS program
Patients may voluntarily opt in to recurring appointment reminders and care updates during authenticated EMR onboarding. The Contact & Support Preferences step presents an unchecked checkbox:
☐ Yes, I agree to receive recurring appointment reminders and care updates from Palm Health at the mobile number I provided.
Consent is not required to receive care. Up to 4 msgs/month. Msg & data rates may apply. Reply STOP to cancel and HELP for help.
How consent works
• The checkbox is unchecked by default.
• The patient actively chooses whether to select it.
• The patient can complete care and portal enrollment without selecting it.
• The patient’s current consent choice is recorded in the EMR.
• No routine SMS is sent without active consent.
• Patients can revoke consent in the portal or reply STOP at any time.
This page reproduces the disclosure shown in the authenticated onboarding flow. It contains no patient information.
